2024 Black Health Equity Coalition of Frederick Survey

Hello! Thank you for participating in the first comprehensive Black health survey of Frederick County. This survey contains questions about all aspects of your health. As such, you can help identify the most prevalent and pressing health issues among Frederick’s Black community by completing this survey. Your answers will help our coalition better understand your needs so that together we can develop plans of action to address key health issues. It will take approximately 10 minutes to complete the questionnaire. Surveys must be completed by September 30, 2024.

Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important for us to learn your opinions. Your survey responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential.
